On behalf of Love City Ministries we extend our sincerest concerns and prayers in your time of hardship.  In recognition of your request for financial assistance, if approved and granted, it is intended to be a one-time gift.  Please respond with the following information:
Eligibility & Approved Contribution:
·   Tithing Partner: Partners who consistently participates in tithing – up to $300
·   Non-Tithing Partner: Partners who do not participate in tithing – up to $150
·   Tithing Ministry Attendee: Persons who tithe consistently, but are not partners- up to $250
·   Non-Tithing Ministry Attendee: Persons who attends but do not participate in tithing- up to $50
·   Community Members (do not attend Love City) see below.

NOTE: Individuals are only eligible to receive benevolence twice in a calendar year.

I hereby acknowledge that I have read and understood the terms of the Benevolence Policy.  I certify that the information provided is accurate to the best of my knowledge. I understand that the decision of Love City Ministries’ board is final, and that misrepresentation may result in disqualification from receiving assistance.
